不同载荷作用下C4~5椎间孔孔径的变化

Space variation of C4-5 bone intervertebral foramina resulting from different loadings
Abstract
AIM: To analyze the pathologic variation of the space of bone intervertebral foramina between C4 and C5 (C4-5) under the different loadings.
METHODS: The experiment was accomplished in the Affiliated Hospital of Xiangnan University between January and May in 2006. Ten fresh adult corpse cervical samples were adopted (volunteer donation or relatives' consent) with intact cervical disc, ligament and the facet joint capsule ranged from C3 to T1 segment. All specimens were divided into 4 groups: C4-5 having undergone cervical intervertebral discectomy (B group) or no cervical intervertebral discectomy (A group) or cervical disc prosthesis replacement (C Group) or anterior cervical discectomy and fusion using bone graft and plate fixation (D Group). The pathologic variational regulation of length diameter, superior width, inferior width, and the space of C4-5 bone intervertebral foramina was analyzed under the different conditions of C4-5 through different graded-loading-forces (25, 50, 75, 100, 125, 150 N, 5 mm/min) in axial on adult corpse cervical spine samples.
RESULT: The space of bone intervertebral foramina between C4 and C5 in B group was significant statistically decreased compared with A, C and D groups [25 N loading: (57.26±17.65), (65.81±16.83), (75.37±17.75), (66.21±16.50) mm2, P < 0.05; 50 N loading: (57.13±17.64), (65.72±16.85), (75.35±17.75), (66.11±16.46) mm2, P < 0.05; 75 N loading: (55.26±17.66), (64.80±16.85), (73.32±17.74), (65.21±16.48) mm2, P < 0.05; 100 N loading: (53.22±17.66), (63.81±16.83), (71.35±17.76), (65.27±16.46) mm2, P < 0.05; 125 N loading: (51.25±17.64), (62.82±16.83), (69.25±17.74), (65.25±16.43) mm2, P < 0.05; 150 N loading: (49.16±17.65), (61.84±16.86), (67.15±17.73), (65.24±16.42) mm2, P < 0.05]. The space of C4-5 bone intervertebral foramina was also higher in C group than in D group (P < 0.05).
CONCLUSION: There has been an obvious decrease in the space of bone intervertebral foramina under the condition of C4-5 having undergone cervical intervertebral discectomy.

摘要
目的:分析不同载荷对C4~5椎间孔孔径变化的影响。
方法:实验于2006-01/2006-05在湘南学院附属医院进行。成人新鲜尸体颈椎标本10具(自愿捐献或家属同意),节段包括C3~T1,剔除肌肉组织,保留椎间盘、韧带和关节囊结构的完整。分别测量椎间盘完整、椎间盘髓核摘除、颈人工椎间盘置换和前路钢板植骨内固定4种状态下,加压速度为5 mm/min时,25,50,75,100,125和150 N的分级轴向载荷加载于标本时C4~5上下径、上前后径、下前后径以及椎间孔面积的变化。
结果:①轴向加载25 N时,C4~5椎间孔面积椎间盘髓核摘除标本小于完整椎间盘、颈人工椎间盘置换和前路钢板植骨内固定标本[(57.26±17.65),(65.81±16.83),(75.37±17.75),(66.21±16.50)mm2,P < 0.05];颈人工椎间盘置换标本大于颈椎植骨融合钢板内固定标本 (P < 0.05)。②轴向加载50 N时,C4~5椎间孔面积椎间盘髓核摘除标本小于其他3组[(57.13±17.64),(65.72±16.85),(75.35±17.75),(66.11±16.46)mm2,P < 0.05];颈人工椎间盘置换标本大于颈椎植骨融合钢板内固定标本 (P < 0.05)。③轴向加载75 N时,上下径:C4~5椎间孔面积椎间盘髓核摘除标本小于其他3组[(55.26±17.66),(64.80±16.85),(73.32±17.74),(65.21±16.48)mm2,P < 0.05];颈人工椎间盘置换标本大于颈椎植骨融合钢板内固定标本 (P < 0.05)。④轴向加载100 N时,上下径:C4~5椎间孔面积椎间盘髓核摘除标本小于其他3组[(53.22±17.66),(63.81±16.83),(71.35±17.76),(65.27±16.46)mm2,P < 0.05]。 ⑤轴向加载125 N时,C4~5椎间孔面积椎间盘髓核摘除标本小于其他3组[(51.25±17.64),(62.82±16.83),(69.25±17.74),(65.25±16.43)mm2,P < 0.05]。⑥椎间孔面积:轴向加载150 N时,C4~5椎间孔面积椎间盘髓核摘除标本小于其他3组[(49.16±17.65),(61.84±16.86),(67.15±17.73),(65.24±16.42)mm2,P < 0.05]
结论:颈椎间盘髓核摘除后C4~5椎间孔有效空间明显减少。

课题背景:颈椎间盘髓核摘除后,改变了颈椎正常的生物力学环境,可对脊柱的正常生物力学性能产生明显的负面影响。本文在体外测量颈椎标本轴向载荷时邻近椎间孔形态的变化,探讨颈椎间盘髓核摘除后颈椎退行性变的理论依据。
相关链接:Bryan颈人工椎间盘由两块表面喷涂生物活性材料的钛合金终板与一耐磨的聚氨基甲酸乙酯核组成,属低磨擦、耐磨损、富有弹性髓核设计,力学稳定性及疲劳试验结果良好。大量临床实验证明Bryan颈人工椎间盘置入后能保持颈椎的节段性运动,维持颈椎间盘的应力分布,使因椎间盘塌陷向椎管内膨出的纤维环和后纵韧带“拉直”而解除其对硬膜囊的压迫,手术间隙的高度可长期有效地得以维持。
同行评价:对于颈椎的生物力学研究多集中于运动分析,而本文重点探讨了下位颈椎在轴向压缩力的作用下C4~5椎间孔直径和面积在正常、髓核摘除、人工椎间盘置换和前路钢板植骨固定4种情况下的变化情况,间接分析了椎间盘的受力情况,如果能够同时分析邻近椎间孔的情况将能更好地对下颈椎的整体受力改变给出报告。
