Comparison of short wave ultraviolet irradiation of two different dosages for firearm injury healing and bacteriostastic effect in soft tissues of limbs

Abstract

AIM: To observe the effect of short wave ultraviolet irradiation of different doses on healing of firearm injury in rabbit limbs.

METHODS: The experiment was performed in the laboratories of Department of Physiotherapy, General Hospital of Chinese PLA and Institute of Radiation Medicine,;;;;;;;;;;;;;;;;;;;;, from September 2003 to September 2004. ¢ÙNinety New Zealand rabbits were divided into 3 groups by random: 30 mJ/cm2 original irradiation group, 60 mJ/cm2 original irradiation group and control group with 30 in each group. ¢ÚThe animal model of perforating wound was established by shooting the strong muscle of right thigh with ¡°54¡± type pistol. Immediately after debridement, the wounds were irradiated under ultraviolet spectroscopy (UV) (surface power of 1 mW / cm2, 85% wave length of 254 nm, 10% as visible light, 5% as other wave length light) for 30 s (30 mJ/cm2) and 60 s (60 mJ/cm2) in the 30 mJ/cm2 original irradiation group and 60 mJ/cm2 original irradiation group respectively. The control group did not receive any intervention. ¢ÛOn the second and the third day after modeling, the wound were treated with intra-wound irradiation, with the dose increasing 30% gradationally. Then the dynamic changes of wound healing were observed every day. The average healing times (AHT) was calculated and the average amount of bacterium (AAB) in the wounded tissue was statistically analyzed at hours 0, 4, 6, 8, 12 and 24 after modeling. ¢ÜThe difference of measurement data was compared with variance analysis.

RESULTS: A total of 90 rabbits were involved in the result analysis. ¢Ùgeneral observation result: Seven days after modeling, wounds in the two groups were drier, with little secretion and lots of fresh granulations. Secretion was more, and dark red granulations and a few necrotic tissues appeared in the control group. ¢ÚAHT: The AHT in the 60 mJ/cm2 original irradiation group (39.2¡À4.2) days was longer than that in the 30 mJ/cm2 original irradiation group [(35.6¡À6.7) days£¬P<0.05]. The AHT in the two irradiation groups were shorter than that in the control group [(52.8¡À8.3) days£¬P<0.01]. ¢Ûamount of bacterium in local wound: 0-6 hous after modeling, there was no remarkable difference among all groups. With the prolongation of time, it increased significantly gradually. The increase was the most significance in the control group. At hours 8, 12 and 24, it was £¨6.241¡À1.607£©¡Á104,(1.632¡À0.472) ¡Á105,(7.997¡À1.252) ¡Á105 piece/g in the 30 mJ/cm2 original irradiation group, £¨6.143¡À0.938£©¡Á104,(1.295¡À0.339) ¡Á105,(5.357¡À1.029) ¡Á105 piece/g in the 60 mJ/cm2 original irradiation group, which were fewer than that in the control group [£¨7.086¡À1.238£©¡Á104,(5.639¡À1.314) ¡Á105,(4.597¡À0.729) ¡Á106 piece/g£¬P <0.05-0.01]. At hours 12 and 24 after modeling, it was fewer in the 60 mJ/cm2 original irradiation group than that in the 30 mJ/cm2 original irradiation group £¨P <0.05£©.

CONCLUSION: The 30 mJ/cm2 and 60 mJ/cm2 irradiations are helpful to promote firearm injury healing in soft tissues of limbs, especially the 30 mJ/cm2 irradiation, but the bacteriostastic activity of 60 mJ / cm2 is stronger.
