滨珊瑚及富血小板血浆复合材料修复兔下颌骨缺损★

课题背景:近年来,国内外学者开始将富血小板血浆与各种骨移植材料复合应用于颌骨缺损的修复,取得了一些肯定的结果,显示了富血小板血浆在骨缺损修复领域良好的应用前景。

应用要点:实验将富血小板血浆与滨珊瑚(天然珊瑚的一种)复合,用于修复兔下颌骨缺损,加入含富血小板血浆材料的缺损区较未加入区有更多的间充质细胞和成骨细胞,并且有更多的新生骨形成。提示富血小板血浆能起到很好的促进骨生长的效果,滨珊瑚与富血小板血浆的复合物能显著促进兔下颌骨缺损的修复。

偏倚或不足:①对血液样本手工血小板计数可能会产生误差,造成实验结果的偏倚。②由于兔骨生长较快,而实验采用的观察时间为2,4,8周,时间间隔较长,造成对兔骨组织生长过程的观察不够连续,对富血小板血浆的加入对骨组织生长所产生的影响也不够明确。


摘要

背景:近年来的一些研究表明自体来源的富血小板血浆可能对骨缺损修复具有促进作用。
目的:观察复合富血小板血浆的滨珊瑚对兔下颌骨缺损的修复作用。
设计、时间及地点:随机分组设计、对照动物实验,2005-07/2006-03在中山大学附属第三医院动物实验室完成。
材料:采用海南产滨珊瑚,孔径为150~200 μm,孔隙率40%~50%,碾磨及筛选出直径200~400 μm大小颗粒。
方法:选择新西兰兔24只,采用自身对照方法,在每只动物的两侧下颌骨体部分别作1处洞穿型骨缺损,大小15 mm×5 mm,随机选择一侧为实验侧,另一侧为对照侧。植入滨珊瑚/富血小板血浆的复合物作为实验组,植入滨珊瑚/贫血小板血浆的复合物作为对照组。
主要观察指标:术后2,4,8,12周通过大体观察、X射线片、组织学染色观察兔两侧下颌骨愈合情况。
结果:新西兰兔24只均进入结果分析。术后2,4,8周,加入富血小板血浆的材料区比较对照侧可以看到更多的间充质细胞和成骨细胞,并且有更多的新生骨形成。术后12周,两侧缺损区植骨床生长没有区别,骨质均成熟和致密。
结论:富血小板血浆能起到很好的促进骨生长的效果,滨珊瑚与富血小板血浆的复合物能显著促进兔下颌骨缺损的修复。

Platelet-rich plasma combined with Porites for repairing rabbit mandibular defects

Abstract

BACKGROUND:
Recent researches reveal that autologous platelet-rich plasma (PRP) may promote the repair of bone defect.
OBJECTIVE: To assess the composite graft of PRP and Porites coral in the repair of mandibular bone defect in rabbits.
DESIGN, TIME AND SETTING: It was a randomized grouping, controlled animal study performed in the Animal Experimental Laboratory of The Third Affiliated Hospital of Sun Yat-sen University from July 2005 to March 2006.
MATERIALS: Porites coral, produced in Hainan Province, with the aperture of 150-200 μm and porosity of 40%-50% was milled and filtrated into particles that the diameter was 200-400 μm.
METHODS: Twenty-four New Zealand white rabbits were selected and a 15 mm×5 mm perforated defect of the mandible was made bilaterally in every rabbit. The bilateral defects were randomly immediately grafted with the composite of Porites/PRP (as the experiment group) or the composite of Porites/platelet poor plasma (as the control group).
MAIN OUTCOME MEASURES: The mandibular healing was evaluated by gross observation, X-ray film and histological dyeing at 2, 4, 8 and 12 weeks postoperatively.
RESULTS: Totally 24 rabbits were involved in the result analysis. More mesenchymal cells and osteoblasts, as well as a greater volume of new bone formation were observed in the graft sites that received PRP than the control at 2, 4 and 8 weeks postoperatively. But there was no difference in the graft growth between the two graft sites at 12 weeks, and the newly formed bone was all dense and mature.
CONCLUSION: PRP can promote bone regeneration. PRP/Porites composite can remarkably facilitate the repair of rabbit mandibular defects. .
