膝骨关节炎模型兔关节液中细胞因子白细胞介素1水平及复方归芪液的干预效应

课题背景:研究表明,白细胞介素1是关节软骨破坏中最重要的一种细胞因子,能促进滑膜细胞的增殖和分化,促进滑膜细胞和软骨细胞合成并释放前列腺素E2及胶原酶,后者能导致滑膜炎症反应和软骨基质的崩解,促进关节炎的发生、发展。因此抑制细胞因子特别是白细胞介素1的产生,对关节炎的治疗是有益的。本实验通过对兔膝骨性关节炎模型动物关节液中细胞因子白细胞介素1水平检测,探讨复方归芪液对骨性关节炎的治疗作用,为其临床应用提供可靠的实验依据。

应用要点:实验结果提示复方归芪注射液对兔膝关节炎的防治作用,可能是通过抑制骨性关节炎细胞因子白细胞介素1的分泌而实现的。

相关链接:目前中医对骨关节炎的研究已深入到微循环、骨内压、自由基、细胞因子等分子水平,在骨细胞学水平上也有不少突破,主要集中在中药复方及相应提取物对成骨细胞、破骨细胞、软骨细胞及成纤维细胞的直接及间接干预上。


摘要
目的:实验表明,白细胞介素1是关节软骨破坏中最重要的细胞因子。实验拟进一步观察兔膝骨性关节炎模型关节液中白细胞介素水平的变化以及复方归芪注射液的干预作用。
方法:实验于2007-04/09在大连市第二人民医院骨科实验室完成。①实验分组:健康清洁级新西兰大白兔25只,6个月龄,体质量2.14~3.20 kg,随机抽签法分为正常组、模型组、生理盐水组、透明质酸组和中药治疗组。每组5只。②实验方法:采用兔膝关节腔内注入木瓜蛋白酶造成骨关节炎的病理模型,正常组、模型组正常饲养,其他3组均于术后第8天给药(右下肢关节腔注射),以后每周2次,共10次。生理盐水组:每次注射9 kg/L 生理盐水0.15 mL;透明质酸组:生理盐水稀释成浓度2 kg/L,每次注入0.15 mL;中药治疗组:每次注入生理盐水稀释液1 kg/L 和复方归芪注射液0.15 mL。③实验评估:于造模后取材,采用双抗体夹心酶联免疫吸附实验法分别测定各组兔膝关节液中白细胞介素水平。
结果:纳入大白兔25只,均进入结果分析。正常组未检出白细胞介素1,模型组、透明质酸组、中药治疗组白细胞介素1均呈高表达,但模型组与正常组比较差异显著(P < 0.01)。透明质酸组、中药治疗组与模型组比较,白细胞介素1水平均降低(P < 0.01),而透明质酸组的作用优于中药治疗组(P < 0.05)。
结论:白细胞介素1在骨性关节炎发病过程及复方归芪注射液治疗中起重要作用。

Effects of compound Guiqi injection on interleukin-1 levels in synovial fluid of rabbit knee osteoarthritis models

Abstract

AIM:Interleukin-1 is a major cytokine in arthrodial cartilage destruction. This study investigated the effects of compound Guiqi injection on changes in interleukin levels in the knee of rabbit osteoarthritis models.
METHODS: Experiments were performed at the Laboratory of Department of Orthopaedics, Second People's Hospital from April to September 2007. Twenty-five healthy clean 6-month New Zealand rabbits of 2.14-3.20 kg were divided into five groups randomly, normal, model, saline, hyaluronic acid and Chinese herb groups with 5 in each group. Genu osteoarthritis models were made by intraarticular injection with caroid. Rabbits were normally fed in the normal and model groups. Administration was performed at day 8 after the surgery in the saline, hyaluronic acid and Chinese herb groups by injection into joint cavity of right lower limbs, twice a week, totally 10 times. 9 kg/L saline (0.15 mL) was injected into rabbits of the saline group. 2 kg/L saline (0.15 mL) was injected into rabbits of the hyaluronic acid group. 1 kg/L saline and compound Guiqi (0.15 mL) were injected into rabbits of the Chinese herb group. Interleukin levels in knee synovial fluid were measured in each group by double antibodies sandwich Enzymelinked Immurosorbent Assay.
RESULTS: Twenty-five rabbits were included in the final analysis. Interleukin-1 levels were not detected in the normal group. Interleukin-1 levels were high in the model, hyaluronic acid and Chinese herb groups There was no significant difference between the model and normal groups (P < 0.01). When comparing hyaluronic acid and Chinese herb groups to the model group, interleukin-1 levels were reduced (P < 0.01). The effect was better in the hyaluronic acid group than in the Chinese herb group (P < 0.05).
CONCLUSION: Interleukin-1 plays an important role in the onset of osteoarthritis and compound Guiqi injection treatment.
