TGF-β3c2s2基因转染骨髓间充质干细胞对创面愈合中转化生长因子β

课题背景:重庆市卫生局医学科技计划资助项目:基因修饰骨髓间充质干细胞创面移植预防增生性瘢痕的实验研究(06-165)。课题前期已成功采用AdEasy-1腺病毒载体系统,通过分子生物学方法构建了含人TGF-β3c2s2目的基因的重组腺病毒载体,获得高滴度的TGF-β3c2s2重组腺病毒。本文以重组腺病毒为载体,用TGF-β3c2s2基因修饰的骨髓间充质干细胞进行创面移植进一步研究其对增生性瘢痕的作用。
相关链接:随着医学材料、组织工程学和干细胞等方面研究的不断深入,加速创面的愈合有了多种新的策略,但创面愈合后胶原过度沉积、增生性瘢痕的形成仍是需要进一步解决的问题。寻求更为有效的能够促进创面愈合、减少病理性瘢痕形成的方法,一直是国内外科研及临床工作者努力的方向。基因治疗和细胞替代治疗是近年来医学领域乃至生命科学领域研究的热点和前沿。重组腺病毒载体是一种能高效转移并表达外源基因的载体系统,所携带的目的基因在宿主细胞内可获得短时大量表达,极适合于具有明确阶段性创伤的愈合和增生性瘢痕的治疗,骨髓间充质干细胞在体内外能有效表达各种治疗性目的基因,所以又被认为是一种基因治疗的靶细胞。在创伤愈合的研究中发现,骨髓间充质干细胞进行创面局部移植能促进创面的愈合。
创新要点:①尽管有较多的干细胞移植及基因转染作为促进创面修复的新方法的文献报道,但严重创伤和深度烧伤的创面愈合后仍然存在较严重的病理性瘢痕。本文采用基因转染骨髓间充质干细胞创面移植的方法,在促进创面修复同时提高减轻病理性瘢痕的基因治疗鲜见报道。②本文结果显示,骨髓间充质干细胞体内诱导分化及表达转化生长因子β3蛋白,对创面修复过程中转化生长因子β1和转化生长因子β3表达有着显著的影响,通过这种影响可能减轻创面修复后增生性瘢痕的形成,为创伤修复早期干预性防治增生性瘢痕的细胞移植和基因治疗提供理论与动物实验基础。
摘要
目的:外伤后病理性瘢痕的形成与创面的愈合过程有着密切的联系,创伤愈合时期引入细胞因子或生长因子及其他影响因素,调节创伤愈合机制,即可能改变细胞外基质结构,达到预防瘢痕形成的目的。实验拟观察TGF-β3c2s2基因转染的骨髓间充质干细胞在创面修复与重塑过程中对转化生长因子β1和转化生长因子β3表达的影响。
方法:实验于2005-10/2007-03年在重庆医科大学附属儿童医院外科实验室、动物实验中心完成。①实验材料:四五周龄日本大耳白兔2只,体质量300~400 g,雌雄不限;健康成年日本大耳白兔20只,体质量1.7~2.5 kg,雌雄不限,均由重庆医科大学动物实验中心提供,实验过程中对动物处置符合动物伦理学标准。②实验方法:选取四五周龄日本大耳白兔分离培养骨髓间充质干细胞。选取健康成年日本大耳白兔20只,每只兔耳腹侧制作2个皮肤、软骨全层缺损创面共80个,创面以自身对照为前提随机分为4组,每组20个创面:实验组:加入Ad-TGF-β3c2s2转染的骨髓间充质干细胞;空白对照组:加入消毒的DMEM/F12(不含胎牛血清);腺病毒组:加入Ad-TGF-β3c2s2腺病毒;骨髓间充质干细胞组:加入骨髓间充质干细胞。③实验评估:观察瘢痕形成情况,苏木精-伊红染色、免疫组织化学技术观察瘢痕组织的组织学形态和检测瘢痕组织中转化生长因子β1和转化生长因子β3表达和变化规律。
结果:①实验组在整个过程中无明显高出周围皮肤的瘢痕形成,其他组上皮化后,创面均逐渐形成不同程度的瘢痕,伤后 45 d 瘢痕增生程度达高峰,明显高出皮肤表面;持续至90 d左右。②实验组和腺病毒组结构较接近正常皮肤,比正常皮肤胶原排列致密,略厚;空白对照组、骨髓间充质干细胞组伤后45 d可见浅层组织结构排列紊乱,胶原纤维粗大呈交错网织状排列。③实验组伤后21,45,90 d转化生长因子β1表达明显低于其他组和正常皮肤(P < 0.01),其表达随时间逐渐下降,转化生长因子β3表达高于其他组和正常皮肤(P < 0.01),各组转化生长因子β3的表达随时间逐渐下降。
结论:TGF-β3c2s2基因转染的骨髓间充质干细胞移植于局部创面,对创面修复过程中转化生长因子β1和转化生长因子β3表达有显著影响,通过这种影响减轻创面修复后增生性瘢痕的形成。

Effects of bone marrow mesenchymal stem cells with TGF-β3c2s2 gene on the expressions of transforming growth factor beta in wound healing
Abstract
AIM: Primarily isolated hepatic oval cells have the potential of bidirectional differentiation and the ductility of transversal differentiation.
OBJECTIVE: To induce the differentiation of rat hepatic oval cells into insulin-producing cells with a chemical inducer.
DESIGN, TIME AND SETTING: A control observational cellular experiment was performed at the State Key Laboratory of Basic Research Department of General Hospital of Chinese PLA from May 2007 to January 2008.
MATERIALS: Specific pathogen free (SPF) SD rats weighting (120±20) g were selected in this study.
METHODS: Hepatic oval cells were isolated using modified gradient centrifugation, and then identified by immunohistochemistry. Subsequently, hepatic oval cells were cultured in RPMI 1640 medium containing Nicotimide and a high concentration of glucose. Simultaneously, hepatic oval cells were cultured in RPMI 1640 medium without Nicotimide or high concentration glucose as a negative control.
MAIN OUTCOME MEASURES: SDS-polyacrylamide gel electrophoresis (PAGE), enzyme linked immunosorbent assay (ELISA) and radioimmunoassay (RIA) were used to detect the insulin expression in oval cells from both groups.
RESULTS: The oval cells isolated by modified gradient centrifugation had the features of stem cells, could self-assemble to form colony. These cells were positive for OV6, CD34 and Nestin. SDS-PAGE, ELISA and RIA demonstrated that insulin levels were significantly higher in the cells cultured in the RPMI 1640 medium containing Nicotimide and high concentration glucose than in the negative controls 1 week after culture (P < 0.05).
CONCLUSION: Nicotimide and a high concentration of glucose can induce hepatic oval cells to directionally differentiate into insulin-producing cells by activating the expression of PDX-1 in a short period.
